1. If you have, COPD, Asthma, Diabetes or Heart Disease
You will have a yearly health check with the practice nurse. The nurse will talk to you about how to manage your health condition including how to use your medicines.
If you are a diabetic you will have an appointment with a health care assistant for a foot check, blood and urine tests before you see the nurse.
When you first join the practice we will ask you book to see the nurse in the first two months.
After this, we will do your health check on your birth month every year. If you have a number of health conditions the nurse will check you for all your conditions.
Please book for this without waiting to be called. If you do forget you will receive a reminder letter.
You will also have a six monthly medicines check with the nurse or doctor to discuss your health condition.
2. If you have regular medicines on repeat prescription
We have a six monthly medicines check system. Please book to see your doctor to have you medicines /health check before the review date on your repeat prescription slip.
When you first join the practice we will ask you to book to see the doctor in the first two months.
After this we will review your medicines every six months. If your condition is stable your doctor may suggest that you have a yearly check.
3. If you have none of the above medical conditions and are aged between 40 – 74 you can book for an NHS health check.
