We provide medical care to patients within the areas of West Meads, North Bersted, Aldwick, Rose Green and central Bognor Regis. If you are unsure whether you are in our catchment area please ask our reception staff.

 

If you are living here temporarily, studying at university, visiting friends or on holiday here then you can register with us as a Temporary Resident. This entitles you to free local healthcare without transferring your records from your usual NHS GP. Contact our reception staff to arrange this.

 

If you have moved here from another area then you will need to register with us. We will then arrange for the transfer of all your old records from your previous GP.

 

If you wish to change from your current GP and transfer your care to West Meads, all you need to do is register with us and we will do the rest.

 

You can pre-register on-line. This sets up your basic details on our computer system and makes completing your registration quicker and simpler. To pre-register all you need to do is complete this easy form.

 

Whether or not you have pre-registered you will need to visit the surgery to complete the process and provide some basic health information. Please bring with you:

·        Proof of your current address (e.g. utility bill, bank statement)

·        Photographic proof of identity (e.g. passport, photo driving license)

·        Name and address of your last GP practice

·        NHS Number (if you are unsure of this ask your last GP practice)

·        Post code of your previous address

·        And don’t forget a list of any medicines that you take regularly (and anything you are allergic to!)

 

We will then arrange transfer of all your records. This can take a few days or sometimes weeks.
